If you need to your Blood Pressure checked during the Pandemic and do not have access to a monitor, you can use a sphygmomanometer, also known as a blood pressure monitor, or blood pressure gauge, which is used to measure blood pressure. This can be purchased at a low cost from online websites.

With immediate effect in line with NHS Digital we no longer have a Fax Machine.

Any patients who are seen in the Practice now need to wear a face covering or mask if asked to attend an appointment. If you exempt then you need to visit the Government Website for an exemption form.

We are now offering a new Video Consultation Service and to use this service you will need to be registered for Patient Access. During these uncertain times, the best way to register for this service is by setting up online as below. If you are unable to do this you can download a form and return to us in the post or email.

Please note any digital image submitted as part of an online consultation, will be stored within your clinical notes.

Patient Registration:  If you are registering as a New Patient, you will need to complete a GMS1 (purple form) and a New Patient Form according to your age.  We can not process any New Registrations without both of these forms.

New Patient Registration

New Registrations are encouraged, to be completed and submitted online.  If this is not possible, you may print and complete manually and post through the Practice mailbox where we will process your Registration without Identity Checks. We would like to confirm that in the current situation, delivery may be by any means, including by post and digital options. A signed, scanned application or picture of a signed application emailed to the practice is acceptable.  If you are unable to complete the form yourself then please contact us and we will be happy to complete on your behalf.

New Patient Registration

We welcome patients from within our practice area

If you would like to register with us, you can either come into reception and collect a Registration form and New Patient Questionnaire or you can download these documents and print by clicking on the links below. Once we have received the forms from you may make your first appointment  5 days after we have received your registration forms.

You will need to download and print a GMS1 form (Family Doctors Services Registration) and a New Patient form according to your age.  We will not be able to process your Registration until both of these forms are complete.

Before your first appointment please bring your completed forms between 11:00 - 15:00 Monday to Wednesday and Fridays so we can register you before your appointment.

We will require two types of proof of identity, one of the following is needed from each Group:

• Passport

• Driving Licence

• Birth Certificate

• ID Card for European Countries

To verify your current address:

• Utility Bill,

• Visa, Rent Book

• Bank Statement

For children under the age of 5 years, we will need you to complete a Health Form for our Health Visitors which you can download and print by clicking on the link above.

All patients will now be registered with ‘The Haven Practice’ not an individual doctor. You will be able to make an appointment with any one of Doctors.

When you have registered with the Practice, we will offer you a New Patient Health check with the nurse.

Our Premises are wheelchair accessible to all areas of the Practice. We have a Portable Induction Loop system available, should you require this facility please let Reception know when you visit the Practice.

We have baby changing facilities at the Practice located in the toilets.
