Any patients who are seen in the Practice now need to wear a face covering or mask if asked to attend an appointment. During your visit, our Toilet facilities will not be available.

We are now offering a new Video Consultation Service and to use this service you will need to be registered for Patient Access. During these uncertain times, the best way to register for this service is by setting up online as below. If you are unable to do this you can download a form and return to us in the post or email.

Patient Guidance for Video Consultations

Please note any digital image submitted as part of an online consultation, will be stored within your clinical notes.

Patient Registration:  If you are registering as a New Patient, you will need to complete a GMS1 (purple form) and a New Patient Form according to your age.  We can not process any New Registrations without both of these forms.

Making an Appointment

Please call in or telephone 01273 555999 to make an appointment. You will usually be seen on the day you request. Babies and children are always seen on the same day.

You can make an appointment with any one of the doctors by:

  • Calling into Reception
  • Contacting us by telephone
  • Booking Online

(You must register in person with the surgery before using Patient Access for the first time. Find out more HERE.)

The Practice philosophy is that we like to take time when seeing our patients. For this reason you may experience delays during some surgeries. The receptionists will advise you of any such extended waiting time.

Telephone Advice

If you feel an appointment unnecessary but would like some advice, please ring and ask to speak to the doctor or nurse who will call you back when they are free (usually at the end of the surgery).

Do not forget to leave your correct contact details.

Specialist Referrals

If you need to see a specialist then please come and see the doctor who will refer you as appropriate.

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.
