Any patients who are seen in the Practice now need to wear a face covering or mask if asked to attend an appointment. During your visit, our Toilet facilities will not be available.

We are now offering a new Video Consultation Service and to use this service you will need to be registered for Patient Access. During these uncertain times, the best way to register for this service is by setting up online as below. If you are unable to do this you can download a form and return to us in the post or email.

Patient Guidance for Video Consultations

Please note any digital image submitted as part of an online consultation, will be stored within your clinical notes.

Patient Registration:  If you are registering as a New Patient, you will need to complete a GMS1 (purple form) and a New Patient Form according to your age.  We can not process any New Registrations without both of these forms.

First Contact Physiotherapists

The Role of First Contact physiotherapists (FCP's) in primary care is to assess patients with soft tissue, muscle and joint pain and support patients to decide on the most appropriate management pathway. You do not need to see your GP first as the FCPs are expert in the assessment of these conditions and are able to request investigations such as xrays and blood tests if it is required. FCP's are physiotherapists with expertise in the assessment and management of Musculoskeletal (MSK) conditions. They may also be known as an Advanced Practice Physiotherapist (APP) or MSK practitioners.

Contact our practice on 01273 555999 to book an initial telephone appointment.
