Any patients who are seen in the Practice now need to wear a face covering or mask if asked to attend an appointment. During your visit, our Toilet facilities will not be available.

We are now offering a new Video Consultation Service and to use this service you will need to be registered for Patient Access. During these uncertain times, the best way to register for this service is by setting up online as below. If you are unable to do this you can download a form and return to us in the post or email.

Patient Guidance for Video Consultations

Please note any digital image submitted as part of an online consultation, will be stored within your clinical notes.

Patient Registration:  If you are registering as a New Patient, you will need to complete a GMS1 (purple form) and a New Patient Form according to your age.  We can not process any New Registrations without both of these forms.

Interpreting Services

If you need an interpreter to accompany you to your consultation to make it easier for you and the Doctor, please ask the receptionist to book an interpreter to accompany you at your appointment.
When patients require the support of an Interpreter at the Practice, we need to ensure we provide the right services. Book a 20 minute appointment where possible with the Clinician.

The interpreting service enables the patient to engage with medical as well as admin staff, to make sure they have a choice and informed decisions around their care and needs.
There are two options when booking an Interpreter:
 Face to Face consultation
 Telephone consultation
For Community Support (Sign Language, lip speaking, Dual sensory loss please contact reception to enable us to arrange support though Action Deafness Communications.

Patients requiring British Sign Language or Braille also need to let us know in advance.

Come anytime between 11.00am – 1.00pm on 1st and 3rd Tuesday of every month. No appointment necessary.
