New Patient Registration

We regret to inform you that with immediate effect that we will be unable to accept any new patient registrations.

We welcome patients from within our practice area

If you would like to register with us, you can either come into reception and collect a Registration form and New Patient Questionnaire or you can download these documents and print by clicking on the links below. Once we have received the forms from you you may make your first appointment 24 hours after we have received your registration forms.

Before your first appointment please bring your completed forms between 11:00 - 15:00 Monday to Wednesday and Fridays so we can register you before your appointment.

We will require two types of proof of identity, one of the following is needed from each Group:

• Passport

• Driving Licence

• Birth Certificate

• ID Card for European Countries

To verify your current address:

• Utility Bill,

• Visa, Rent Book

• Bank Statement

For children under the age of 5 years, we will need you to complete a Health Form for our Health Visitors which you can download and print by clicking on the link below.

All patients will now be registered with ‘The Haven Practice’ not an individual doctor. You will be able to make an appointment with any one of Doctors.

When you have registered with the Practice, we will offer you a New Patient Health check with the nurse.

Our Premises are wheelchair accessible to all areas of the Practice. We have a Portable Induction Loop system available, should you require this facility please let Reception know when you visit the Practice.

We have baby changing facilities at the Practice located in the toilets.
